Market Engagement – Complex Needs Pathway - Joint Health and ***** Services | The Lakeview Room | Tuesday, 18. September 2018

Worcestershire County Council is looking to commission a new service to provide a Complex Needs Pathway in Worcestershire. The pathway is intended to provide services across health and social care.
We would like, at this tentative stage in the development of proposals, (and prior to formulating any formal procurement opportunity), to seek input from the market as to what the appetite might be for undertaking these services.
We are aiming to review and monitor patients and service users known to present with a high risk of harm to self and/or others and who are likely to require intensive support to manage relapse and behaviour that challenges services, in order to prevent in-patient admission and improving outcomes for individuals. Identified individuals will have been assessed and met one or more of the following criteria to be registered on the complex needs pathway:

Individuals with a recent history of multiple placement breakdown
Individuals presenting a high risk of admission/readmission (e.g. S117, CTO, Guardianship) to locked and secure services
Individuals with a high risk of offending or reoffending
Individuals with a Mild LD and MH diagnosis who have a high risk of relapse requiring specialist intervention
Complex individuals whose placements are not considered sustainable due to high risk of harm to self and others
Individuals currently in in-patient locked and secure services
Complex individuals with a diagnosis of LD and/or ASD who present with behaviours that challenge services

The County Council intends to hold an open information and feedback session for all interested organisations.
We are looking for sustainable and skilled providers with appropriate experience/ evidence of effectively managing complex individuals, and who are interested in developing services in Worcestershire to meet the complexity of this identified group.
